A model study found a missing upper left central incisor, space loss, and crowded lower arch. Treatment aimed to strengthen oral cleanliness, preserve overbite, minimize overjet, align teeth, maintain Class I relationships, and make room for the incisor.

Making space for missing middle incisor (4)

author: Andreas Tjandra | publisher: drg. Andreas Tjandra, Sp. Perio, FISID

Model study revealed a missing upper left central incisor, which is linked to space loss (3 mm), and a little crowded lower arch (4 mm). The patient's occlusion included a 7 mm overjet, an average and incomplete bite on the palatal mucosa, and midlines that coincided between the upper and lower teeth. Special examinations of the upper incisors revealed normal tooth color and normal cold and percussion test responses.

The problem list is as follows:

  • Skeletal pattern of class II
  • Maxillary incisors that are inclined
  • Overbite to soft tissue that is incomplete
  • Increased overjet of 7 mm
  • On the left is a ½ unit Class II molar connection, whereas on the right is a full unit.
  • Class II canines
  • Space loss of the upper left central tooth
  • Crowded lower arch.
The goals and objectives of the treatment were to:
  • Strengthen oral cleanliness
  • Preserve overbite
  • Minimize overjet
  • Align and level teeth, correct positioning.
  • Attain Class I relationships for incisors and molars.
  • Make room for the upper left central incisor.
  • Retention.

Summary

Model Study on Oral Health Issues

  • Found missing upper left central incisor, linked to space loss and crowded lower arch.
  • Patient's occlusion included a 7mm overjet, incomplete bite on palatal mucosa, and midlines overlapping between upper and lower teeth.
  • Problems: Class II skeleton pattern, inclined maxillary incisors, incomplete overbite, increased overjet, Class II canines, space loss of upper left central tooth, and crowded lower arch.
  • Treatment goals: Strengthen oral cleanliness, preserve overbite, minimize overjet, align and level teeth, maintain Class I relationships, make room for upper left central incisor, and retain.
